Radical Transparency: Cara Kirsch’s Mission to Fix Healthcare Benefits
In this episode of Triple Margin Freedom, I sit down with Cara Kirsch, founder and CEO of The Kirsch Group, to talk about one of the most expensive and least transparent decisions most leaders make: employee health benefits.
Cara has spent nearly 30 years on every side of the table—HR, carriers including UnitedHealthcare and Blue Cross Blue Shield of Nebraska, and brokerage. In late 2025, she left a long career and launched her own independent firm because she was finished with the industry’s quiet conflicts. Carriers pay bonuses. Incentive trips happen. Fees stay buried. Employers sign large checks without a clear picture of who their broker actually works for.
Her answer is radical transparency paired with real fiduciary responsibility. She refuses carrier incentives. She discloses fees and conflicts the way a retirement-plan advisor is required to. She treats benefits as a co-fiduciary duty, not a once-a-year spreadsheet. Combined with better data, technology, and a people-first culture—remote work, paid leave, high trust—The Kirsch Group serves employers from roughly 50 to 6,000 employees with strategy, independent RFPs, fiduciary oversight for benefits committees, and education that turns complexity into usable decisions. The firm aligns with Health Rosetta principles: the client’s financial interest comes first.
We also cover Cara’s leadership arc. Early on she led with more command-and-control. Today she holds high expectations and pairs them with compassion. A decade ago she founded the Ignite conference to support women in insurance; it has since grown into a broader women-in-business gathering, with its 10th event planned for 2027. She is building toward an employee-owned firm and a team of about 20.
The practical conversation is pointed. Healthcare costs keep rising under pressure from AI-enabled billing, new procedures, and inertia. Employers who run a disciplined RFP, demand full fee transparency, and treat their advisor as a fiduciary partner, protect both margin and people. Benefits are not just an HR line item. They are a leadership choice that affects profit, time, and whether your best people stay.
